In the case of advanced prostate cancer, surgery combined with radiation therapy yields better results.

New research on prostate cancer shows that the primary therapy for localized and advanced prostate cancer with radical prostatectomy and adjuvant radiation therapy is associated with a lower risk of cancer-related mortality and an increase in overall survival, compared to primary therapy involving radiation therapy and androgen deprivation.

 

However, radical prostatectomy and adjuvant radiation therapy are associated with higher risks of erectile dysfunction and urinary incontinence.

 

Researchers from the Cancer Institute of New Jersey, led by Dr. Thomas Zhang and his colleagues, conducted a study on 13,856 men aged 65 and older who were diagnosed with localized and advanced prostate cancer.

 

During a 14.6-year follow-up, 2,189 deaths were recorded, of which 702 were related to prostate cancer.

 

Despite cancer stage and Gleason score, the prostate cancer-specific 10-year survival rate and overall 10-year survival rate were higher with radical prostatectomy plus radiation therapy (RT-XRT) than with radiation therapy plus androgen deprivation (XRT-ADT). This was published online by Dr. Zhang’s team before being printed in the journal Cancer.

 

For example, in patients with a prostate cancer diagnosis of T3a-bN0-XM0, the prostate cancer-specific 10-year survival rate was 88.9% in the RT-XRT group, while in the XRT-ADT group, it was 74.2%. The overall 10-year survival rate was 64.2% and 48.3%, respectively.

 

The RT-XRT group showed a higher incidence of erectile dysfunction (28% vs. 20%) and a higher percentage of urinary incontinence (49% vs. 19%).

 

The researchers state that the interpretation of their conclusions within the scope of the study should be limited. Factors such as administrative requirements, radiation dosage, and others could have influenced the results.
